🛡️ Ontario Family Court Form 25 (Restraining Order) Checklist

A simple step-by-step guide to make your restraining order official and enforceable by police.

✅ Step 1: Prepare the Draft

  • Fill out Form 25: Restraining Order.

  • Include all details: No Contact, Stay Away addresses, and Police Enforcement Clause.

  • Double-check accuracy.

📌 Tip: Get help from a family lawyer or duty counsel if possible.

✅ Step 2: Judge Reviews & Signs

  • Bring your draft to your court hearing.

  • Judge may:

    • Sign as is

    • Cross out terms

    • Add new conditions

  • Once signed, it’s an official court order.

✅ Step 3: Court Clerk Issues the Order

  • Clerk stamps the order with the court seal.

  • Request several certified copies.

✅ Step 4: Register with Police

  • Take a certified copy to your local police station.

  • Ask them to enter it into the Canadian Police Information Centre (CPIC) database.

  • This ensures all police across Canada can enforce it immediately.

✅ Step 5: Always Carry a Copy

  • Keep one copy of the order with you at all times.

  • If you call 911, having the document speeds up enforcement.

🔒 Key Reminders

  • Without police registration, your order cannot be enforced.

  • Be very specific about exceptions (e.g., parenting communication).

  • Orders can be temporary or final, depending on what the judge grants.

  • Violating a restraining order is a criminal offence — call police immediately if it happens.
